Whenever a driver has lost or destroyed his/her village issued vehicle for hire license, he/she shall submit an affidavit in writing to the Village Administrator, stating that the license has been lost or destroyed. The Village Administrator shall issue a duplicate copy or substitute thereof upon payment of $5 for each item so duplicated or substituted.
