A verified application for a vehicle for hire driver's license shall be made in writing to the Village Administrator upon forms provided by the village along with a payment of $100 for application or renewal thereof. The application shall state the full name of the applicant, his/her age, his/her residence address, the name and address of his/her employer, and whether or not he/she qualifies for a village issued vehicle for hire license under this chapter. The applicant must undergo a state criminal background check and must complete any forms required by the state to undergo such a background check.
