§ 110.021 APPLICATION FOR LICENSE.
   (A)   An application for a license shall be made in writing to the Local Commissioner and shall be signed by the applicant and verified under oath.
   (B)   The application shall be in a form and shall contain information as may be required by the Local Commissioner and shall be accompanied by the applicable nonrefundable application fee and the annual fee for the license.
