Within 30 days after a permit to collocate a small wireless facility on a municipal utility pole has been issued, the village and the applicant shall enter into a Master Pole Attachment Agreement provided by the village. The Master Agreement shall govern the terms for the initial collocation on a municipal utility pole by the applicant. For subsequent approved permits to collocate a small wireless facility on a municipal utility pole, the village and the applicant shall enter into a License Supplement of the Master Pole Attachment Agreement.
