Application is hereby made for approval of the final plat. The following documents are made a part of this application:

   For major subdivisions:

      a.   Twelve copies of the final plat.

      b.   Twelve copies of protective covenants and restrictions.

      c.   Twelve copies of an agreement, executed by the subdivider to construct improvements in accordance to the improvement plans.

      d.   One copy of financial guarantees as required in § 152.022(B).

      e.   Final plat fees. Refer to § 152.022 for applicable fees.

   For minor subdivisions:

      a.   Fourteen copies of final plat.

      b.   Fourteen copies of protective covenants and restrictions.

      c.   Fourteen copies of an agreement, if necessary, executed by the subdivider to construct improvements in accordance to the improvement plans.

      d.   One copy of financial guarantees as required in § 152.022(B).

      e.   Final plat fees. Refer to § 152.022 for applicable fees.
