§ 111.03  ISSUANCE OF LICENSE.
   (A)   Any person desiring to operate a food establishment shall make a written application for a license on forms provided by the Health Department. Such application shall include the name and address of each applicant, the location and type of the proposed food establishment, and the signature of each applicant.
   (B)   Prior to approval of an application of a license, the Health Department shall inspect the proposed food establishment to determine compliance with the requirements of this chapter.
   (C)   The Health Department shall issue a license to the applicant if its inspection reveals that the proposed food establishment complies with the requirements of this chapter.
   (D)   Licenses for permanent food establishments shall expire one year following the date of issuance. Licenses for temporary food establishments shall be issued for a period of time not to exceed 14 days.
   (E)   There may be a fee charged for the license for a food establishment. The fee shall be collected by the Health Department at the time a new license is renewed, or issued, and shall be deposited into the Health Department Fund. The fee schedule shall be as approved by the Board of Health. (Prior approval must be granted by a majority of the County Board regarding a reasonable food license fee schedule prior to implementation by the County Board of Health).
