The Health Authority may permanently revoke a permit for serious or repeated violations of this chapter, or for interference with the Health Authority in the performance of its duties. The Health Authority shall notify the permit holder of its intent to revoke the permit, stating the reason for the action and that the permit shall be revoked at the end of five business days following the serving of the notice. Said revocation shall remain valid pending a hearing request submitted by the permit holder in writing to the Health Authority within the five day period.
