161.02 QUALIFICATIONS.
   (a)   To qualify for all compensation and benefits hereinafter provided, each full- time, part-time and special employee employed after April 1, 1977, shall have completed and submitted an employment application form prior to the commencement of employment. The Mayor shall cause a central file to be established, maintained, and administered for all applications for "unclassified" jobs.
   (b)   Each department head shall review the employment application forms for all qualified applicants prior to employing a person to fill a job in his department. The employment application form of each person employed, bearing the approval of the appropriate department head, shall be filed in the employee's personal file.
