771.03 INVESTIGATION OF APPLICANT; INSURANCE.
   The Mayor shall investigate each applicant to determine the character, business responsibility and suitability to render the services of a trash hauler within the City. The applicant shall furnish and file with the Mayor the following:
   (a)   The name of each person in the employ of the hauler and a personal history statement of each, and any and all other information as deemed necessary by the Mayor.
   (b)   A policy or policies of liability insurance issued for the life of the license applied for or longer, by a responsible insurance company, approved as to sufficiency by the Mayor and as to legality by the Law Director, providing indemnify for or protection to the applicant against loss resulting from the operation of each vehicle to the extent of one million dollars ($1,000,000) on account of injury or death of one person in any one accident; two million dollars ($2,000,000) on account of injury or death of more than one person in any one accident; and one hundred thousand dollars ($100,000) for property damage caused in any one accident.
