Permits issued pursuant to Section 959.01 shall be issued in triplicate. One copy shall be retained by the Director of the South Euclid-Lyndhurst Recreation Board, one shall be delivered to the City of Lyndhurst and one shall be issued to the applicant. Each permit shall include the name of the organization requesting the permit, the date the permit is issued, the field to be used, the time the field may be used under the permit and the name of the person in charge. The permit shall also contain the signature of the Director.
