Every applicant for a license shall file with the Mayor a written application stating the full name and address (or names and addresses, if the number of applicants is more than one) or, if an association or firm, the full names and addresses, including streets and numbers, of the members of such association or firm. If the applicant is a club, society or corporation, the application shall contain a complete list of the officers of such club, society or corporation, with their names and addresses, and shall also give the name of the state under which they are organized and the names of one or more persons who are designated as managers, with their addresses. The application shall also state the following:
(a) The premises where the billiard or pool room is to be conducted, with the street and number.
(b) The age of the applicant, in the case of individuals, and the age of the manager, in the case of a firm, society or corporation, with their full addresses.
(c) Whether or not the applicant or manager is a citizen of the United States.
(d) Whether or not the applicant has ever been engaged in operating a billiard or pool room, and, if so, when and where, and the length of time within three years last past.
(e) The name of the owner of the premises where such billiard or pool room is to be located, with his or her complete address. The application shall be signed by the applicant or applicants, or, in the case of a firm, society or corporation, by the manager or an officer.
