Upon payment of the business permit fee herein assessed by the town and made payable to the town clerk, the clerk shall then issue to the applicant a certificate reflecting the following data:
A. The name of the person, association, firm or corporation paying such fee.
B. A description of the nature of the business in which the applicant shall be engaged.
C. The date such fee is paid.
D. The address of the person, association, firm or corporation paying such fee.
E. The address where the business physically shall be located.
F. The amount paid for such fee. (1987 Code § 5.04.100)
