(a) All persons required to file a Village Income Tax Return pursuant to Codified Ordinances 171.01 through 171.99 shall be required to attach to the same a completed form “Alternative to 1040" substantially similar to the example attached to Ordinance 3227 made part hereof and marked “Exhibit A”.
(b) This form may be amended from time to time by the Tax Commissioner. If however no amendment is made by the Tax Commissioner then the use of the form attached hereto as “Exhibit A” is mandatory.
(c) Nothing in this ordinance shall repeal or limit any other provision within Codified Ordinances 171.01 through 171.99.
