(a) Any eligible employee of the Village who waives health care benefits under the plan provided by the Village for its full-time employees shall receive a monthly incentive payment equal to twenty-five percent (25%) of the savings incurred by the Village due to said waiver, or one hundred twenty-five dollars ($125.00), whichever is less.
(b) Said written waiver of health care benefits must be signed by the employee and personally delivered to the office of the Clerk/Treasurer in order for the waiver to be in effect. (Ord. 4067. Passed 6-9-22.)
