The application shall be approved by Metro Government upon a finding that the following criteria have been met:
(A) The applicant has obtained a permit to use the public way pursuant to § 115.091;
(B) The applicant has in force a Commercial General Liability insurance policy as specified in § 115.094; and has provided a certificate of insurance, or complete copy of the policy if requested, to IPL; and
(C) The applicant has registered with, and maintains good standing with, the Louisville Metro Revenue Commission.
