(a) Application for a permit for a composting operation shall be made to the City of Lorain on forms approved for such purpose. Applicants shall provide the following information, at a minimum:
(1) Name, address, and telephone number of the applicant;
(2) The telephone number(s) of the owner or operator who can receive notice in the event of emergency;
(3) The address of the facility sought to be permitted;
(4) Approval of the Planning Commission, as required by the City of Lorain;
(5) Evidence of required state or local permitting or licensing;
(6) Site sketches with buildings, storage bins, unloading/loading areas, and fencing labeled on the sketch;
(7) Operating hours;
(8) List of materials accepted;
(9) Operating plan which includes methods used to screen for unauthorized material, how material will be handled, description of any processing and a schedule for removal of materials; and
(10) Any other information requested by the City of Lorain.
(b) It is a condition of the permit that all information in the permit application be kept current. Any change in required information shall be reported to the Safety Service Director, on a form provided by the department, no later than ten business days after such change has occurred.
