An application for an adult use facility or sexually oriented business shall be made on a form provided by the city clerk and approved by the city council. This application shall include:
A. A detailed statement of all activities and types of business to be conducted on the premises. The statement shall be in sufficient detail to determine the extent to which said business fits the definitions of "adult uses" or a sexually oriented business.
B. The address and legal description of the land or property on which the adult use or sexually oriented business shall conduct business.
C. A building plan, drawn to scale, of the premises to be built, leased, or renovated. Said building plan shall include existing configuration and final configuration of floor plans.
D. Intended days and hours of operation.
E. Provisions made to restrict access by minors.
F. Provisions made to provide security for employees and patrons. (Ord. 2006-217, 8-3-2006, eff. 8-3-2006)
G. A signature to a statement attesting to the fact that any withholding of facts or information or the providing of false information through the course of the application and investigation process and subsequent operation of the business shall be grounds for denial or revocation of the business application. H. The full name, street address, city, state and zip code of the applicant or applicants, and if a corporation, partnership or fictitious business entity, then the full names and current street addresses of all owners, partners, or persons who hold any type of equity ownership position or managing authority in the company.
