Each application for an amusement device license shall set forth the name and address of the owner, the name and address of the exhibitor, the address of the place of business at which the amusement device to be licensed is to be located, the calendar year for which the amusement device license is sought and a description of the amusement device to be licensed, including the name, make, model and any other identifying information, and shall be submitted to the Mayor for approval.
