§ 52.31 INSTALLATION.
   (A)   A customer of the Municipal Water Department may be required by the Cross-Connection Control Officer to install and maintain a properly located backflow prevention device at the customer’s expense appropriate to the potential hazards set forth in 179 Neb. Admin. Code § 2, “Regulations Governing Public Water Supply Systems” and approved by the Cross-Connection Control Officer.
   (B)   The customer shall make application to the Cross-Connection Control Officer to install a required backflow prevention device on a form provided by the municipality. The application shall contain, at a minimum, the name and address of the applicant, the type of backflow device to be installed, including make and model number, and the location of the proposed installation.
   (C)   The Cross-Connection Control Officer shall approve or disapprove the application based on whether such installation will protect the municipal water distribution system from potential backflow and backsiphonage hazards.
