§ 114.04 APPLICATION FOR PEDDLERS, SOLICITORS, AND CANVASSERS.
   An application for a peddler's, solicitor's, or canvasser's license shall be made by each business entity, as well as each person causing or intending to solicit, peddle or canvass with the city to the City Clerk in conformity with the general requirements of city ordinances relating to the application for license. The application herein required shall be fully completed and signed under oath and contain the following information:
   (A)   Business entity application. The application for the business entity which shall be completed under oath and signed by an officer or executive shall include:
      (1)   Full name, address, telephone number of the business entity;
      (2)   Number of years in existence;
      (3)   Names, addresses and titles of all officers of the entity;
      (4)   Whether the business entity is a charitable organization and, if so, whether it is recognized as such by the Internal Revenue Service and the State of Illinois;
      (5)   Full name, date of birth, addresses and telephone numbers of person(s) representing the entity who will directly supervise the peddlers, canvassers, or solicitors in the city;
      (6)   A description of the merchandise or goods to be peddled or offered for sale or order, or the purpose and nature of the materials to be canvassed;
      (7)   Whether a license to the business entity has ever been suspended or revoked;
      (8)   A written representation by the business entity, that it has instructed persons conducting the peddling, soliciting and canvassing on its behalf not to perform such activities at any residence or business which has a sign posted in the following or similar words “No Solicitors, Peddlers, or Canvassers”;
      (9)   Full name, date of birth, address, telephone, social security and driver's license numbers of all persons who will be conducting the solicitation, canvassing or peddling on behalf of the business entity in the city;
      (10)   A written statement which is specifically designated to be relied upon by the city and its residents that within a month prior to submitting its application the business entity has personally and fully conducted an investigation on the background of each person the entity purposes to conduct the canvass, solicitation or peddling on its behalf and that each such person is a law abiding person of good moral character and has not been convicted of a felony or crime of moral turpitude within ten years from the date of this application; and
      (11)   The first day upon which the entity would like to peddle, solicit or canvass in the city. The completed application must be filed not less than three business days prior to the first date requested.
   (B)   Application for individual. The application for the person who will peddle, solicit or canvass shall be completed and signed under oath by the applicant and shall include:
      (1)   The full name, date of birth, address, social security and driver's license number of the applicant;
      (2)   The full name, date of birth, address and telephone number of the applicant's employer and the name and address of applicant's supervisor;
      (3)   The kind of merchandise or goods to be peddled or offered for sale or order, or the purpose and nature of the matters to be canvassed;
      (4)   The number and description, including make, model and license plate numbers of vehicles, if any, intended to be operated in the city by the person in the course of soliciting, canvassing or peddling;
      (5)   The name of all cities, villages or towns where applicant has engaged in peddling, soliciting, or canvassing within the last two years;
      (6)   Whether the applicant, or his or her employer, principal of the entity which is represented by the applicant, has ever had a peddler's, solicitor's or canvasser's license revoked and/or been convicted of violating any statute or ordinance relating to the same;
      (7)   Whether applicant has ever been convicted of a felony or crime of moral turpitude, including fraud, forgery, theft or deceptive practices, and, if so, the case disposition, case number, offense charges, offense convicted of and date and place of such conviction; and
      (8)   An acknowledgment there shall be no canvassing, peddling or soliciting at any business or residence which has signs posted in the following or similar words “No Solicitors, Peddlers or Canvassers.”
The applicant shall state the exact first date the applicant desires to peddle, solicit or canvass. The completed application must be filed at least three business days prior to the first date requested.
