Application to conduct a sidewalk sale shall be made in writing to the City Clerk by a representative of the person seeking the permit at least 30 days prior to the date of the event, and shall set forth the following information:
(A) The name, address, and telephone number of the person or organization seeking to conduct the sidewalk sale.
(B) If the sidewalk sale is proposed to be conducted for, on behalf of, or by an organization, the name, address, and telephone number of the headquarters of the organization.
(C) The name, address, telephone number, and position or title, if any, of the person in charge of the sidewalk sale.
(D) The date or dates, and the hours on the dates, when the proposed sidewalk sale is to be held.
(E) The location of the sidewalk sale.
(F) The amount of space on the sidewalk to be occupied by the apparatus connected with the sidewalk sale.
