§ 91.30 REQUESTS FOR ISSUANCE OF PERMIT TO EXCAVATE.
   No work shall be made in any street, alley, sidewalk, curb, or gutter owned by or under the control of the city without first securing a permit from the city and no permit shall be issued without either the owner of the property adjacent to the street desired to be worked or the person, firm, or corporation who proposed to make the excavation, first filing with the city the following:
   (A)   Where the total valuation of the work to be done exceeds $300, a performance bond with a corporate surety or a cashier's check payable to the city equal to 50% of the total valuation of the work to be done, which guarantees the replacement of the street, parkway, sidewalk, curb and gutter, and the street pavement in a neat, workmanlike manner as herein provided.
   (B)   Where there is work in a street or alley, evidence of a public liability and property damage insurance policy in force in limits of $500,000 injury or death to any one person and $1,000,000 injury or death as the result of any one accident and $100,000 property damage naming the city as an insured.
   (C)   Evidence that the person doing the work has barricades available to barricade the street excavation during the progress of the work with his name and address printed thereon.
   (D)   Pay a fee based upon the total valuation of the work to be done in the following amounts:

      (1)   Each additional $1,000 or fraction thereof and including $20,000, $5 per $1,000 valuation.
      (2)   Each additional $1,000 or fraction thereof from $20,000 to $50,000, $4 per $1,000 valuation.
      (3)   Each additional $1,000 or fraction thereof from $50,000 to $100,000, $3.50 per $1,000 valuation.
      (4)   Each additional $1,000 or fraction thereof from $100,000 to $1 million, $2.50 per $1,000 valuation.
      (5)   Each additional $1,000 or fraction thereof over $1 million, $1.25 per $1,000 valuation.
