§ 51.133 MANNER OF REQUESTING HEARING.
   (A)   A standard form of requesting an administrative hearing pursuant to this subchapter shall be available at the office of the city of Lockport during all regular business hours maintained by that office.
   (B)   The hearing request form shall include adequate space for any person requesting a hearing to provide the following information.
      (1)   The name of the person requesting the hearing;
      (2)   The address of the person requesting the hearing;
      (3)   The address where service is provided, if different;
      (4)   The telephone number of the person requesting the hearing;
      (5)   A summary of the nature of the complaint or challenge to the pending administrative action; and
      (6)   A description of previous interaction or contacts with the city concerning the complaint or challenge to the pending administrative action.
