(A) Notice of the availability of an administrative hearing pursuant to this subchapter shall be given to all persons being issued a shut-off notice for water and sewer service.
(B) Such notice shall include the following:
(1) That any person may challenge the impending administrative action, whether termination of service or institution of collection proceedings, or other action, by requesting an administrative hearing before the Hearing Party pursuant to this chapter;
(2) That a request for hearing may be made by completing a standard form available at the office of the city of Lockport within eight days of the mailing by the city of the notice of impending administrative action, such disconnection of service, institution of collection proceedings, or the like; and
(3) That failure to request an administrative hearing will result in the threatened action being taken within ten days of the mailing of the notice of such pending action.
