149.05   SICK LEAVE; HOSPITALIZATION AND MAJOR MEDICAL.
   (a)   Sick leave. An employee shall be a probationary employee for six months from the date of hiring.
      (1)   Full-time employees of the Village shall be entitled to fifteen days sick leave with pay each year employed, which shall be credited at the rate of one and one-quarter (1-1/4) days per month.
      (2)   Full-time employees of the Village may use sick leave for absence due to illness; pregnancy; injury; exposure to contagious disease which could be communicated to others; and for absence due to illness, injury, or death in the employee's immediate family.
      (3)   “Immediate family” shall be interpreted to include father, mother, grandparents, brother, sister, husband, wife, child, parent-in-law, or anyone who has stood in the same family relationship with the employee as any of the above.
      (4)   A full-time employee of the Village who transfers from another public agency in Ohio shall be permitted to transfer the amount of his or her accumulated sick leave up to a maximum accumulation allowable by the Village.
      (5)   Sick leave shall be cumulative to a maximum of 200 days.
      (6)   Beginning January 1. 1999, each full-time employee will receive five days of unexcused absence per year. After five days are missed during the year a doctor’s statement is needed if an employee is to be paid for any additional absences. In addition, a doctor’s statement will be required of an employee who misses more than three consecutive work days. The note must be signed and dated. Light duty assignments do not exist: the employee is either fit to return to work or is not fit to return to work. In cases of extended illnesses, one note for period of absence will be used. The total number of sick days will not be changed unless the sick days are used. The doctor’s statement must be presented to the Clerk- Treasurer in order for the sick day compensation to be paid. It is the employee’s responsibility to see that this is done upon the first day returned to work.
      (7)   The Mayor may at any time require of an employee an affidavit (notarized statement) as to the reason for his/her use of sick leave.
   (b)   Hospitalization and major medical premiums.  The Village shall pay 90% hospitalization and major medical premiums for all full-time employees except for those employees who were previously at the base pay or above. In lieu of these employees not receiving a raise the village shall pay 100% of their hospitalization In addition, the Village shall pay 100% hospitalization and major medical premiums for the Clerk-Treasurer.
