Every application for a license shall be made to the City Clerk on forms supplied by the city and shall state the applicant's name and address, the name of the business, if different, the business or activity for which a license is sought, the address of the premises on or from which the business is to be operated or the activity is to be conducted, any other businesses operated from or on the same premises, and any other information as may be required by this code or by the Council. All applications shall be accompanied by payment in full of the license fee. The license fee shall be refunded if the license is denied, unless otherwise specifically provided in this chapter. All licenses must be approved by the Council, except when otherwise authorized by this code.
