Each application for a permit must include all of the following information in detail before it can be accepted by the City Secretary and referred to the appropriate city or county official or agency:
(A) The identity and residence address of the applicant;
(B) The identity and residence address of the owner of the land where the facility will be located;
(C) The location and legal description of the land where the facility will be located;
(D) A positive statement that any city, county, school district ad valorem taxes have been paid and are current concerning the land where the facility will be located;
(E) A positive statement that no private residences and no church or school are within 300 feet of the proposed facility as measured in the most direct manner;
(F) A statement as to proposed days and hours of operation; and
(G) A statement as to safety precautions to be in place for the safety of patrons of the facility as to fire protection requirements and police accessibility.
