§ 111.16 APPLICATION.
Applications for the licenses shall be made to the village president in writing, signed by the applicant, if an individual, by a partner or an agent if a partnership, or by a duly authorized agent thereof if a club or corporation, verified by oath or affidavit. The application shall be submitted with a nonrefundable application fee in the amount set forth in the comprehensive fee schedule set forth in Section 12.02 of this code and said application shall contain the following statements and information:
   (A)   The name, date of birth, social security number, driver’s license number and address of the applicant and if a partnership in addition thereto the same information for all partners and if a corporation, in addition thereto the same information for the proposed manager of the premises, each officer, each director and any stockholder owning more than 25% of its outstanding stock.
   (B)   The citizenship of the applicant, his place of birth, and if a naturalized citizen, the place and date of naturalization.
   (C)   The character of business of the applicant; and in the case of a corporation, the objects for which it was formed and attach a copy of its articles of incorporation.
   (D)   The length of time the applicant has been in business of that character, or in the case of a corporation, the date when its charter was issued.
   (E)   The amount of goods, wares and merchandise on hand at the time application is made.
   (F)   The location and description of the premises or place of business which is to be operated under the license.
   (G)   A statement whether the applicant has made application for a similar or other license on premises other than those described in this application, and the disposition of the application.
   (H)   A statement that the applicant has never been convicted of a crime and if he or she has, list the convictions, law violated, jurisdiction, date of conviction and disposition.
   (I)   State whether a previous license by any state or subdivision thereof or by the federal government has been denied or revoked, and if so, the reason therefor.
   (J)   A statement that the applicant will not violate any of the laws of the village, state or United States in the conduct of his place of business.
