(A) No permit shall be issued except on written application to the Village Clerk in the form prescribed by the Chief of Police. All applications shall contain the provision that the Village shall not be liable for any failure of service or damage that might result from the installation or operation of the alarm system.
(B) The form of application for an alarm user shall be prepared by the Chief of Police and shall contain the following information: name of applicant, address of the residence or business in or upon which the alarm system has been or will be installed, telephone number, type of alarm system, name of the person, firm or corporation selling, installing, monitoring, inspecting, responding to and/or maintaining the alarm system; the name, address and telephone number of 1 or more persons who can be reached at any time and who is authorized to respond to an alarm signal and open the premises in which the system is installed.
