§ 116.03   APPLICATION FOR LICENSE.
   Any individual, corporation, or partnership desiring to operate any of the institutions covered by this chapter shall first comply with all regulations and state statutes relating to those institutions whether so denominated by state provisions or not, and then such party shall make application for a license to so operate those institutions by filing with the Finance Director a written request for a permit setting forth the following information:
   (A)   The location or proposed location of the nursing home or boarding home;
   (B)   The purpose for which it is to be opened;
   (C)   The number of rooms to be occupied;
   (D)   The number of admittees in each room, and whether such admittees will be adults or children or both;
   (E)   The name and address of the physician to be retained should there be one in the care of a boarding home and all cases in a nursing home;
   (F)   The name and address of the agency placing such admittees, if there be any; and
   (G)    The name and address of the person or agency responsible for the support of the admittees, if there be any.
