§ 50.25 APPLICATION REQUIREMENTS.
   (A)   Certificate of compliance applications shall be made on forms provided by the Department and signed by the applicant and an appropriately certified practitioner, including the practitioner’s certification number and date of expiration.
   (B)   The applications shall include the items listed below:
      (1)   Name, mailing address, telephone number of the owner, engineer, and installer;
      (2)   Property identification number and address or other description of the property;
      (3)   A design and engineer report prepared by a state-registered engineer;
      (4)   Design data, percolation tests, or soils analysis test and one copy of the plan, specifications, and other pertinent information pertaining to supplies and equipment to be used in construction of the on-site wastewater treatment system;
      (5)   A certified statement that the on-site wastewater treatment system is designed in conformity with applicable state law and regulations and the requirements of this chapter; and
      (6)   Other relevant information requested by the Department as necessary to properly evaluate the application.
