Applicants for the license shall disclose under oath the following:
(A) The name and address of the applicant. If the applicant be a corporation, the address of the principal place of business of the corporation, and the name and address of all of the corporate officers;
(B) The names of all persons who are in any manner financially interested in the billiard tables; and
(C) The names of all employees who will work on the premises where the billiard tables are located.
