Any person requiring a license shall make application therefor in writing to the Building Commissioner on a form to be prescribed by the Building Commissioner. Such application shall contain the name and address of the applicant and shall state whether the applicant expects to engage in billposting or sign painting or in the construction, erection, maintenance or alteration of signs or outdoor display structures and such other relevant information as the Commissioner may require.
