(a) Applications for an ambulance license shall be made upon such forms as may be prepared or prescribed by the License Officer and shall contain such information as the License Officer shall deem necessary to administer or effectuate the purposes of this chapter; and shall contain at least the following information: a description of the ambulance, including the make, model, year of manufacture, motor and chassis number, the current State or Federal Aviation Agency license number, the mileage and length of time the ambulance has been in use, and a description and photograph of the color scheme, insignia, name, monogram or other distinguishing characteristics to be used to designate the ambulance. The applicant shall provide the License Officer a copy of their current State Board of Emergency Medical, Fire, and Transportation Services Ambulance License and a copy of their most recent Ohio Department of Public Safety, Division of Emergency Medical Services, periodic mechanical safety report for each ambulance to be licensed. The mechanical safety report must have been completed within the previous six calender months. Applicants are exempt from having to provide a mechanical report on any ambulance that is less than one year old. A fee in the amount specified in Section 868.01 shall accompany the application for each license.
(b) In addition, to receive and maintain a license, each ambulance shall, at all times when in use as such:
(1) Be suitable for the transportation of patients with regard to health, sanitation and safety and be maintained in suitable premises;
(2) Contain equipment conforming to the standards, requirements and regulations provided for in this chapter, which equipment shall be in proper and good condition for such use;
(3) Comply with all applicable laws and local ordinances relating to health, sanitation and safety;
(4) Be equipped with such lights, sirens, special markings and other equipment or apparatus as may be required by State law or regulation;
(5) Have an insignia, logo, and/or color scheme as approved by the License Officer to designate it as an ambulance;
(6) Be equipped with original equipment manufactured-approved safety belts and harnesses for the driver and for a passenger in the front seat which shall be worn pursuant to State and local laws;
(7) Be equipped to provide advanced life support with a minimum of one EMT, EMT-B, or EMT-I and one paramedic;
(8) Be covered by and maintain proof of insurance as required in this chapter.
(c) Any change in the ownership of an ambulance licensed under this chapter shall terminate the license and shall require a new application.
