(a) Applications for an emergency medical service organization (EMSO) license shall be made upon such forms as may be prepared or prescribed by the License Officer and shall contain at least the following information:
(1) The names, social security numbers, dates of birth, and addresses of the owner(s) of the EMSO;
(2) The corporate, trade or other fictitious name, if any, under which the EMSO does business or proposes to do business;
(3) The experience of the EMSO in providing 911 Emergency response services, primary advanced life support service, and the transportation and care of patients;
(4) A description of the ambulances, including the make, model, year of manufacture, motor and chassis number, the current State or Federal Aviation Agency license number, the mileage and length of time the ambulances have been in use, and a description and photograph of the color scheme, insignia, name, monogram or other distinguishing characteristics to be used to designate the ambulances;
(5) The location and a description of the place or places from which the EMSO is intended to operate;
(6) Such other information as the License Officer shall deem necessary to administer or effectuate the purposes of this chapter;
(7) An accompanying fee as specified in Section 868.01.
(b) To receive and maintain a license, each EMSO must meet all of the following requirements:
(1) It shall have at least two or more fully equipped advanced life support, paramedic-manned, licensed ambulances, staffed twenty-four hours a day, seven days a week, 365 days a year.
(2) It shall also have one or more fully equipped advanced life support ambulance(s) stationed within the City limits that can be placed into service in the event one of the two primary ambulances are temporarily out of service.
(3) It shall have two primary ambulances with no greater than 250,000 vehicle miles.
(4) It shall have no ambulance in service with over 350,000 vehicle miles.
(5) It shall have adequate facilities for housing ambulance vehicles in a climate- controlled environment and shall have clean and suitable facilities for all office and field personnel. The location of the place or places from which the EMSO is intended to operate shall be centrally located within the City.
(6) It shall have certified emergency medical dispatchers (EMD) or EMT dispatchers and facilities continuously and effectively staffed twenty-four hours a day, seven days a week, 365 days a year.
(7) It shall respond to emergency code number three calls within a maximum of nine minutes after accepting such a call and respond to non-emergency code number one calls within a maximum of fifteen minutes after accepting such a call, with compliance rate of at least ninety-five percent over any three-month period for each type of call.
(8) It shall have a medical director from either Lima Memorial Hospital or St. Rita's Medical Center in Lima, who is a board certified physician and possesses at least five years experience in emergency medicine.
(9) It shall have and maintain proof of insurance as required in this chapter.
(c) An EMSO shall notify the License Officer of any change in personnel within two working days of the change.
