Application for such license shall be made to the City Clerk on a form supplied by the city. The application shall state:
(A) The name, address, date of birth and social security number of the applicant and of all persons associated with him or her in his or her business.
(B) The type of business for which the license is desired.
(C) In case of transient merchants, the place where the business is to be carried on.
(D) The length of time for which the license is desired.
(E) A general description of the thing or things to be sold.
(F) The place of residence of the applicant for the five years next preceding the date of application. Every application shall be accompanied by a $50 application fee, which amount shall be credited on the license fee if the license is granted. If the license is denied, the application fee is non-refundable. Every application shall bear the written approval of the Police Department or Mayor after an investigation of the applicant's business reputation and moral character.
