Excavation permits may be issued by the City Clerk upon written application therefor, on forms to be supplied by the city, by the person doing the digging or making the excavation or by the person for whose benefit the digging or excavation is being made; provided, however, that the permit shall be issued in the name of the benefited person or property owner.
