In order to obtain a license to operate a coin-in-the-slot amusement device described in section 4-2-1 of this chapter, within the village limits, the applicant shall file with the village clerk a written statement setting forth the full name and address of the applicant or applicants, or the names and addresses of the stockholders of the corporation if the applicant is a corporation, or the names and addresses of the officers of any club or association, and the present or last occupation of the applicant or applicants as of the time of the filing of the application. Said application shall be signed by the applicant, or in the case of a firm, corporation, club or association, an authorized signatory who shall file, along with the application, proof of his or her authorization, which application shall be referred by the village clerk to the mayor of the village.
