4-1-6: APPLICATION FOR LICENSE:
Application for such license shall be made to the mayor in writing upon blanks prepared and furnished by the village, signed by the applicant, if an individual; or by at least two (2) members of a partnership, if the applicant is a partnership; or if the applicant is a corporation, then by the president and secretary thereof; verified by oath or affidavit of the signers, and shall contain the following information and statement:
   A.   The name, age and address of the applicant in the case of an individual; in the case of a copartnership, the persons entitled to share in the profits thereof; and in the case of a corporation, for profit, or a club, the date of the incorporation, the objects for which it was organized, the names and addresses of the officers and directors, and if a majority interest of the stock of such corporation is owned by one person or his nominees, the name and address of such person.
   B.   The citizenship of an applicant, his place of birth, and if a naturalized citizen, the time and place of his naturalization.
   C.   The character of business of the applicant; and in case of a corporation, the objects for which it was formed.
   D.   The length of time that the applicant has been in the business of that character; or in the case of a corporation, the date on which its charter was issued.
   E.   The amount of goods, wares, and merchandise on hand at the time application was made.
   F.   The location and description of the premises or place of business which is to be operated under such license.
   G.   A statement whether applicant has made similar application for a similar or other license on premises other than described in this application.
   H.   A statement that applicant has never been convicted of a felony and is not disqualified to receive a license by reason of any matter or thing contained in this chapter or any ordinance or statute. (Ord. 87-1, 4-7-1987, eff. 4-7-1987)
   I.   Whether a previous license by any state or subdivision thereof, or by the federal government, has been revoked, and the reasons therefor, or whether the applicant has made an application for a liquor license which has been denied and, if so, the reasons therefor.
   J.   A statement that the applicant will not violate any of the laws of the state of Illinois or of the United States or any ordinance of the village in the conduct or his business.
   K.   That the proposed location is not within one hundred feet (100') of any church, school, hospital, home for aged or indigent persons or veterans, their wives or children, or any military or naval station. (Ord. 87-1, 4-7-1987, eff. 4-7-1987; amd. 2003 Code)
   L.   The date of incorporation, if an Illinois corporation; or the date of becoming qualified under the Illinois business corporation act 1 to transact business in Illinois, if a foreign corporation.
   M.   That he has not received or borrowed money or anything else of value, and that he will not receive or borrow money or anything else of value, except as permitted by 235 Illinois Compiled Statutes 5/1 et seq. (2003 Code)
