A. Any person seeking to conduct or operate a raffle shall file an application therefor with the Village Clerk on forms provided by the Village Clerk. Such application shall contain the following information:
1. The name and address of the organization and evidence it is a qualified applicant under section 4-7-5 of this chapter.
2. The name, address and telephone number of the organization's presiding officer and Raffles Manager.
3. The maximum price to be charged for each raffle chance issued or sold.
4. The geographic area of areas in which raffle chances will be sold or issued.
5. The maximum number of days during which raffle chances will be determined.
6. The times and locations at which winning chances will be determined.
7. The certificate signed by the presiding officer of the applicant organization attesting to the fact that the information contained in the application is true and correct. (Ord. 05-16, 7-5-2016)
