Except for work exempt under subsection 7-2-1B of this chapter, a written application shall be made for any permit, which application shall be made in duplicate. No specific form must be used for the application, but it must state the nature and extent of the work sought to be performed. The application must be accompanied by a plat, tracing, sketch or drawing showing the location, character and dimensions of the proposed opening(s), including, but not limited to, alterations involving changes in the location of pipes, conduits, wires or conductors.
