Each application shall indicate the name of the property owner or owners upon whose property the water transmission line shall be constructed, the name of the Kentucky registered engineer or surveyor preparing the plans, a copy of the plans and specifications which shall have been approved by the appropriate state and federal agencies, and the name and address of the contractor.
