§ 91.46 SURFACE RESTORATION.
   All streets that are to be cut or excavated for installation of underground utility lines of whatever kind and nature will be restored by permittee, as follows:
   (A)   Bituminous surfacing:
      (1)   Fill earth will be tamped or compacted to an elevation 18 inches below grade;
      (2)   Dense grade aggregate crushed stone will be placed on top of earth fill to an elevation six inches below grade;
      (3)   Concrete will be poured in excavated area on top of crushed stone to an elevation two inches below grade, and which shall extend laterally six inches on either side beyond the principally excavated area;
      (4)   Poured concrete shall be covered by rolled or compacted bituminous surfacing two inches in thickness to finished grade;
      (5)   Disturbed street shoulders shall be replaced with dense grade aggregate crushed stone three inches in thickness and extending three feet from edge of bituminous surfacing.
   (B)   Crushed stone surfacing;
      (1)   Fill earth will be tamped or compacted to an elevation 12 inches below grade;
      (2)   Dense grade aggregate crushed stone will be placed on top of earth fill 12 inches in thickness to finished grade.
