(A) All applications for licenses shall include, but not be limited to, the following: the name and business address of the applicant, the names and addresses of all persons who have a financial interest in the business, the name in which the business will be conducted and the address of the hotel to be licensed.
(B) All applications shall be signed personally by the applicant, and if the applicant is not a natural person, then by an authorized representative of the applicant.
