(A) The City Controller shall issue a permit for the operation of the amusement devices upon application therefor and payment of the licensing fee. The application for the permit shall state the name and address of the owner or lessee of the amusement device, the name and address of the place of business wherein it shall be operated and the total number of the amusement devices to be located therein.
(B) Every permit issued under the provisions of this chapter shall be numbered and shall bear an expiration date and the serial number and description of the machine for which it was issued.
(C) Every permit must be posted in a conspicuous place in the establishment where the machine is operated.
