(A) All applications for licenses or permits required by this code shall be submitted on forms provided by the Department of Public Works, and the applicant shall provide all information requested thereon.
(B) All applications shall be filed with the Department of Public Works, which shall examine the applications for completeness and compliance with the requirements of this code and shall issue permits to all applicants who qualify.
(C) Failure to completely and accurately provide the information required in any permit application, or the giving of false or misleading information, intentional or otherwise, shall be grounds for denial or revocation of a permit.
(D) Failure of any applicant to comply with any of the qualifications of applicants or duties of licensees prescribed in this code shall likewise be grounds for denial or revocation of a permit.
