(A) With respect to any ordinance violation, the arresting officer shall notify the alleged offender by presenting him or her with a written citation.
(B) In the case of vehicles, if the offender is not present, notification shall be made by posting the written citation in a conspicuous place upon the vehicle.
(C) All citations shall be numbered serially, shall be accounted for by the Violations Clerk and shall contain or state the following:
(1) The make and year of the vehicle involved, if any;
(2) The state of issuance and number of the vehicle license, if any;
(3) The name and address of the violator, if ascertainable;
(4) The driver’s license number of the violator, if ascertainable;
(5) The ordinance violated;
(6) The specific violation charged;
(7) The location, date and time of the violation;
(8) The name, badge number and division, if any, of the officer issuing the citation;
(9) Schedule of fines;
(10) Directions as to the manner in which any fine may be paid;
(11) The places at which the fines may be paid;
(12) A blank signature line following an appropriate written appearance, waiver of trial and plea of guilty; and
(13) A brief warning of the consequences should the alleged violator ignore the citation.
