§ 1-2-5-11. EXTENDED LEAVES OF ABSENCE.
   (A)   An employee may, in certain extenuating, unusual or uncontrollable circumstances, receive an authorized leave of absence without pay, but without loss of seniority, for a period not to exceed one year.
   (B)   To qualify for an extended leave of absence, an employee must submit an application in writing to the employee’s department head, stating the circumstances and reasons for the request.
   (C)   The leave may be granted only upon approval of both the department head and the Board of Public Works and Safety.
