A. Form; Required Information: Application for license hereunder shall be filed, in writing, with the city clerk-treasurer in any form which contains the following information:
1. The names and addresses of the applicant, and if a firm, corporation, partnership, or association, the principal officers thereof and their addresses.
2. The address of the premises where the licensed devices are to be operated, together with the character of the business being carried on at such place.
3. The trade name and general description of the devices to be licensed, the name of the manufacturer and serial number of devices to be licensed.
4. The name and address of the operator of the devices, if other than the proprietor.
B. Fee: The proper license fee, in the form of cash, certified check or money order, shall accompany such application.
C. Referral To Mayor And Council; Investigation: Application for license hereunder shall be first referred by the city clerk-treasurer to the mayor and council who shall make or cause to be made such investigation as they deem necessary. (Ord. 1977-3, 7-18-1977)
D. Issuance: If the application is approved by the mayor and council, the license shall be issued by the clerk-treasurer. (Ord. 1977-3, 7-18-1977; amd. 2007 Code)
E. Denial: If the license is denied, the fee shall be returned to the applicant.
F. Appeal: If an applicant or other interested party contests action taken by the city with respect to any application, a hearing may be requested in the manner set forth with respect to revocation of licenses as provided below. (Ord. 1977-3, 7-18-1977)
