§ 70.33 PERMIT REQUIREMENTS.

   The permit shall include the following information:

   (A)   Describe the purpose of the use of the oversized vehicle;

   (B)   Provide the exact dimensions of the oversized vehicle as provided in the definitions section of this subchapter;

   (C)   Provide a copy of the permit application submitted to the state’s Department of Transportation for an oversize/overweight permit and a copy of the issued permit from the state’s Department of Transportation;

   (D)   Provide a map of the requested route to be used;

   (E)   Provide the date and time the roads will be requested for use in accordance with the requirements in division (I)(1) below;

   (F)   Provide a list of all utility companies that will be affected by the route, signed and dated by a representative of the utility company, stating that the route has been inspected and that there is no problem, or that a representative will be available on the date of usage;

   (G)   Prepayment of an escort fee of $2,500 to the town to cover the cost of four hours of police and public works escort service travel on the town roads. An additional $500 per hour will be assessed for every hour, or fraction thereof, over four hours required for the escort service. No refund will be made on the $1,500 escort fee if less than four hours is required;

   (H)   Submission of a performance bond in the amount of $10,000; and

   (I)   The permittee shall be required to conform with the following:

      (1)   The transport of an oversized vehicle shall be scheduled on a Tuesday, Wednesday or Thursday (except for holidays) to begin between the hours of 9:00 a.m. and 12:00 noon. Other times and days will not be permitted unless prior approval has been granted by the Town Manager. Permits will not be valid during inclement weather and/or dangerous road conditions.

      (2)   The permittee shall have an arborist with an ISA certification available to accompany the oversized vehicle to correctly move, trim or cut any limbs, branches or trees required by the move.

      (3)   Escort vehicles shall be provided in front of and behind the oversized vehicle.
